2010-16 ; Device Approval ; Equipment and Supplies ; European Union ; R158 ; United States Food and Drug AdministrationAbstract: Nieuwe medische hulpmiddelen met een hoog risico voor de patiënt, zoals een nieuw type hartklep of heupprothese, komen in Europa vaak veel vlugger op de markt dan in de VS. Reden is dat de fabrikant in Europa geen bewijs moet leveren dat de patiënt even goed of beter geholpen is met het nieuwe hulpmiddel, dan met de standaard aanpak van het probleem. Hierdoor kan de patiënt wel vlugger gebruik maken van de nieuwe technologie, maar heeft hij ook meer kans blootgesteld te worden aan nog onbekende risico’s. Het KCE onderzocht, samen met een aantal Europese zusteragentschappen, welke soort onderzoeksresultaten fabrikanten moeten voorleggen voordat zij zulke implantaten en andere nieuwe technologieën op de markt mogen brengen. Momenteel zijn de vereisten voor klinische studies veel lager dan die voor geneesmiddelen, en de gegevens over de uitgevoerde onderzoeken zijn weinig of niet toegankelijk.
Het rapport beveelt aan dat de klinische veiligheid en werkzaamheid eerst voldoende zouden moeten aangetoond worden, liefst met vergelijkende studies van hoge kwaliteit. Le test de jugement situationnel permet de jauger les actions et les décisions prises par les candidats dans la sphère professionnelle. Chaque question dépeint une situation professionnelle et une suite de quatre actions possibles. Le candidat devra choisir l’action qui est selon lui, la plus efficace et celle qui l’est le moins.
Pour réussir le test, le candidat doit appréhender chaque situation comme un jeu de rôle en oubliant ses acquis professionnels. Il doit apporter une réponse à chaque question en ne considérant que les données du scénario et les attentes que l’on peut avoir à son égard en termes de compétences et de bonnes pratiques professionnelles. Il doit être capable de hiérarchiser les actions en se servant des critères les plus adéquats : degré de résolution du problème, degré de priorité, positions de vie…
Les concours d’entrée dans les institutions européennes sont très sélectifs. Pour réussir, le test de jugement situationnel, il faut faire preuve de méthode et s’entraîner.

Innovative high-risk medical devices, such as new types of heart valves or hip prostheses, become available on the European market more rapidly than in USA. This is due to the European legislation allowing early marketing of innovative high-risk medical devices before high-quality clinical evidence is obtained from randomized controlled trials.
Methods
We studied the premarket clinical evaluation of innovative high-risk medical devices in Europe compared with the USA. We also discussed patient safety and the transparency of information. The literature and regulatory documents were checked. Representatives from industry, competent authorities, notified bodies, ethics committees, and health technology assessment agencies were consulted.
Results
In contrast to the US, there is no requirement in Europe to demonstrate the clinical efficacy of high-risk devices in the premarket phase. For the patient, this implies earlier access to innovative technology, but at the risk of potential safety issues. At this moment, European requirements for clinical studies are lower for medical devices than for drugs, and data from premarket clinical trials are scarce or remain unpublished. The European Medical Device Directives are currently being reworked.
Conclusions
For innovative high-risk devices, and while awaiting a reworked Medical Device Directive, patient risk should be minimized by limiting the market introduction of novel high-risk devices with minimal clinical data to physicians with the necessary training and expertise.
