L’étude s’est intéressée exclusivement aux soins infirmiers à domicile, et non à la prise en charge globale des patients à domicile. Une réforme du financement postule une réflexion globale sur les rôles attendus de chaque type de services de santé. Pour cela, une meilleure coordination entre les niveaux de compétence politique est nécessaire. Le mode de financement mixte actuellement en vigueur peut être maintenu à condition d’être optimalisé. La nomenclature devrait être actualisée et une distinction plus nette entre soins post-aigus et soins chroniques est souhaitable. De même, la mesure de la dépendance des patients et la qualité des soins devraient jouer un rôle plus accentué dans le financement. De studie behandelt enkel het vraagstuk van thuisverpleging, niet het grotere verhaal van de thuiszorg. De financiering aanpassen vraagt een globale reflectie over de rollen van verschillende gezondheidszorgfuncties. Daarbij is er meer coördinatie tussen de verschillende politieke bevoegdheidsniveaus nodig. De huidige gemengde financiering kan worden voortgezet als ze wordt geoptimaliseerd. De nomenclatuur zou best worden geactualiseerd en er zou een scherper onderscheid moeten worden gemaakt tussen postacute en chronische zorg. Ook het meten van de zorgafhankelijkheid van de patiënt en de kwaliteit van zorg zouden bij de financiering een meer prominente rol moeten spelen. An important question is whether the financing principles can be based on a patient case-mix. This study is limited to homecare nursing as currently reimbursed at the Belgian federal level through the National Institute for Health and Disability Insurance (NIHDI). Home help services, social services, general practitioners, occupational therapists, physiotherapists, as well as community nursing activities such as preventive mother and child care, psychiatric care, midwifery, school health nursing and occupational nursing are beyond the scope of this study. Financing is studied from the provider perspective. Patients accessibility and equity are not addressed. This report describes the financing of home nursing in Belgium as in 4 selected European countries. It also discusses measures of patient dependency in Activities of Daily Living, currently used for case-mix financing. Finally, it reports Belgian stakeholder opinions on issues related to the future financing of home nursing in Belgium. Een antwoord was al gegeven in tal van partiële analyses die wij in het verleden maakten en waar wij vooral wezen op de vele mogelijkheden voor de verdere uitbouw van de thuiszorg. In deze studie actualiseren wij dit antwoord op de vraag, en zijn onderliggende vraag naar verdere ontwikkelingsmogelijkheden. De thuisverpleging is niet los te zien van de ontwikkeling van andere thuiszorgdiensten, die wij naast de thuisverpleging mee in beeld brengen. Ook de institutionele zorg, het alternatief van thuiszorg, is in detail bekeken voor de ouderenzorg.
Deze verkenning van de evidentie gebeurde op basis van bestaande informatiebronnen over de activiteiten in de sector van de thuisverpleging en thuiszorg, maar ook op basis van microdatabanken als de individuele gegevens beschikbaar binnen de ziekteverzekering en de drie golven van de gezondheidsenquête. Deze bronnen lieten toe zowel vraag als aanbod naar de thuisverpleging beter te begrijpen, en ze te situeren in een subregionaal, regionaal én internationaal perspectief. Het rapport concludeert dat de thuiszorg en de thuisverpleging, die een behoorlijke leeftijd hebben bereikt, uiteraard niet meer onderontwikkeld zijn in onze goed uitgebouwde verzorgingsstaat. Er zijn echter nog tal van verdere ontwikkelingsmogelijkheden in onze vergrijzende samenleving, die zij prioritair ook zouden moeten krijgen.Record link: https://kce.docressources.info/index.php?lvl=notice_display&id=2023 Hold
