La meilleure manière de fournir cette aide est d’utiliser une approche intégrée sans créer des structures séparées, estime le Centre fédéral d’expertise des soins de santé (KCE), dans une étude menée en collaboration avec l’Université d’Anvers. La Belgique aurait besoin de 15 entités structurées de manière intégrée pour assurer cette aide, avec au moins une par province. Cette aide d’urgence doit être disponible à tout moment et de préférence apportée de manière aussi proche que possible du milieu de vie de l’enfant ou de l’adolescent. Although evidence-based practice is advocated, the widespread implementation of this kind of practice remains a challenge. This is also the case for evidence-based practice related to the prevention of pressure ulcers, which varies widely in process and outcome in Belgian hospital care. One major obstacle to bridging this knowledge-to-action gap is data availability. We propose using large-scale hospital administrative data combined with the latest evidence-based methods as part of the solution to this problem. METHOD: To test our proposal, we applied this approach to pressure ulcer prevention, using an administrative dataset with regard to 6030 patients in 22 Belgian hospitals as a sample of nationally available data. Methods include a systematic review approach, evidence grading, recommendations formulation, algorithm construction, programming of the rule set and application on the database. RESULTS: We found that Belgian hospitals frequently failed to provide appropriate prevention care. Significant levels of underuse, up to 28.4% in pressure ulcer prevention education and 17.5% in the use of dynamic systems mattresses, were detected. Figures for overuse were mostly not significant. Misuse couldn't be assessed. CONCLUSIONS: These results demonstrate that this approach can indeed be successfully used to bridge the knowledge-to-action gap in medical practice, by implementing an innovative method to assess underuse and overuse in hospital care. The integrative use of administrative data and clinical applications should be replicated in other patient groups, other datasets and other countries. WC 245 Meningococcal infectionsAbstract: Un vaccin contre les infections à méningocoque (du groupe B) a récemment été développé. Cette bactérie est surtout dangereuse pour les nourrissons et les jeunes enfants. En collaboration avec l’Université de Bristol, le Centre Fédéral d’Expertise des Soins de Santé (KCE) a évalué l’impact clinique de la vaccination ainsi que le rapport entre les coûts d’une éventuelle vaccination et ses avantages en termes de santé publique. Les résultats de l’étude montrent que l’introduction de ce vaccin dans le calendrier vaccinal des enfants ne devrait pas être recommandée à ce jour parce qu’il n’aurait qu’un impact faible sur le nombre de cas et de décès, tout en provoquant beaucoup d’effets secondaires. De plus, il subsiste des incertitudes sur sa durée de protection et sur les fluctuations possibles de la maladie dans le futur. Ces conclusions devront être revues lorsque des données plus solides seront disponibles. À l’heure actuelle, aucun pays européen n’a encore intégré ce vaccin dans son calendrier national de vaccination, mais l’Angleterre et le Pays de Galles envisagent de le faire, à condition que son prix diminue.
