WI 610 Anus. Rectum -- NeoplamsAbstract: Le cancer de l’intestin est le cancer le plus fréquent après celui du poumon et de la prostate chez l’homme et après celui du sein chez la femme. Environ 30 % de ces cancers prennent naissance dans la dernière partie de l’intestin, le rectum. Le Centre fédéral d’Expertise des Soins de santé (KCE) avait développé des recommandations pour le traitement de ce cancer en 2007. Il publie aujourd’hui une mise à jour sélective sur base des données scientifiques les plus récentes, réalisée en collaboration avec de nombreux cliniciens et des représentants de patients. Pour la détermination de la taille et de l’étendue de la tumeur, il est recommandé de faire appel à la résonance magnétique (IRM). Sur le plan du traitement, l’intervention chirurgicale est le plus souvent inévitable, mais dans les stades débutants, l’ablation de la tumeur peut être suffisante, ce qui améliore la qualité de vie du patient. Objective: We investigated whether regional variation in cancer incidence was associated with variations in thyroid disease management.
Design: We conducted a retrospective population-based cohort study that involved linking data from the Belgian Health Insurance database and the Belgian Cancer Registry to compare thyroidrelated procedures between regions with high and low cancer incidence.
Main Outcome Measures: Primary outcome measures were rates of TSH testing, imaging, fineneedle aspiration cytology (FNAC), and thyroid surgery. Secondary study outcomes were proportions of subjects with thyrotoxicosis and nodular disease treated with surgery, of subjects treated with surgery preceded by FNAC or with synchronous lymph node dissection, and of thyroid cancer diagnosis after surgery.
Results: The rate of TSH testing was similar, but the rate of imaging was lower in the low incidence region. The rate of FNAC was similar, whereas the rate of surgery was lower in the low incidence region (34 [33; 35 ] vs 80 [79; 81 ] per 100 000 person years in the high incidence region; P _ .05). In the low incidence region compared to the high incidence region, surgery represented a less chosen therapy for euthyroid patients (47 [46–48] vs 69 [68–70]; P _ .05), proportionally more surgery was preceded by FNAC, more cancer was diagnosed after total thyroidectomy, and thyroid cancer patients had more preoperative FNAC and synchronous lymph node dissection.

Belgium ; Cost-Benefit Analysis ; Journal Article ; Magnetic Resonance ImagingAbstract: Against the Belgian background of national planning of MRI units and a detailed reimbursement scheme, a study was undertaken to investigate the cost and productivity of MRI units in different investment scenarios and for various operational schedules. This article focuses on capital costs, not operating cost. Using data from a hospital survey and manufacturers, total capital costs per examination were simulated. The survey revealed considerable variation in operational hours, with on average 66 hours per week, resulting in 6 300 examinations per year per unit. Whilst operational hours remained approximately unchanged over the last 8 years, the number of examinations per unit grew by on average 6% per year. Correspondingly, average examination time declined from 45 to 31 minutes. The findings suggest that, mainly due to the increased productivi ty, capital costs per examination decreased considerably in the examined period. In 2008, the average capital cost per examination is estimated to vary from €23 to €45 for 1.5 Tesla units and from €32 to €62 for 3 Tesla units, assuming an equal examination speed for both types of units.
