Dexia a confié à la Nouvelle Fabrique des Territoires une étude sur les recompositions hospitalières en France survenues au cours des 10 dernières années (1995-2005) . Sans prendre parti dans les débats sur les réorganisations de l'offre hospitalière, cette étude vise à donner des réponses et à établir des faits. Elle retrace l’histoire de 2 100 établissements hospitaliers français de court séjour, décrivant pour chacun d'eux les capacités de l’offre et les opérations de recomposition survenues. "Un premier résultat de cette étude est donc de fournir une base de données inédite, et sans commune mesure, par son exhaustivité, avec les diverses enquêtes précédemment conduites, qui sont toutes restées partielles. Son analyse permet d’apporter des réponses non équivoques à bien des questions fréquemment posées : les recompositions ont-elles été nombreuses ? Y a-t-il une géographie des recompositions ? Quelles sont les caractéristiques des territoires et des établissements concernés ? Le secteur public est-il en retard ? Ces restructurations ont-elles aggravé l’accessibilité aux soins ? Ont-elles créé des problèmes d’emploi ? Quel a finalement été le rôle des ARH alors que l’on s’apprête à créer des ARS ? Deux constats sont posés : 1. des recompositions nombreuses, qui reflètent des stratégies de mutation différentes entre secteur public / privé non lucratif et secteur privé commercial ; 2. des recompositions hétérogènes sur le territoire, qui n'ont répondu que partiellement aux besoins de rationalisation de l'offre de soins. Information storage and retrieval Indexation Classification ; Databases, Factual ; Diagnosis-Related Groups ; Hospitals ; Patients ; Software Abstract: 3M™ APR DRG Software is widely used today for adjusting large volumes of data to reflect severity of illness and risk of mortality. 3M APR DRGs are an extension of the basic DRG structure that includes four severity-of-illness levels and four risk of mortality levels within each DRG. The 3M APR DRG severity and mortality subclasses are assigned according to a sophisticated clinical logic that simultaneously evaluates the interactions of multiple co-morbidities, age, procedures, and principal diagnosis. Patients in each of the four clinically meaningful severity levels experience similar resource utilization and outcomes within the 3M APR DRG assignment. Unlike the CMS DRGs, which focus on the Medicare population, the 3M APR DRGs reflect the complete cross-section of patients seen in an acute care setting. The newborn and pediatric portions of the 3M APR DRGs were developed in conjunction with the National Association of Children’s Hospitals and Related Institutions (NACHRI). The 3M APR DRG Software allows for comparison of hospital service lines, individual physician performance, and patient outcomes through the use of severity adjustment that creates a common measurement across disparate patient types.
