2007-03 ; Child Development ; Journal Article ; Mental Disorders ; Morbidity ; Peer Review ; R97Abstract: Background Few co-morbidity studies have been conducted since the Leeds Consensus Statement on developmental co-ordination disorder (DCD) in 2006. In this Statement, international cut-offs and inclusion criteria were agreed and consequently, the status of DCD changed. Furthermore, most existing co-morbidity studies are small clinical studies, rather than epidemiological studies, resulting in a broad range of co-morbidity rates. DCD has a higher incidence for boys in comparison with girls; questions arise if this preponderance remains the same in combination with other developmental disorders. Therefore, in this study we aimed to determine co-morbidity and gender differences of motor problems in children with a pervasive developmental disorder, a hyperkinetic disorder and/or a speech, language or learning disability. Methods Profiles of 3608 children (mean age: 9 years 1 month) referred to rehabilitation centres for behavioural, developmental and sensorineural disorders were studied. Results Motor problems were reported in one-fifth of the total sample. Co-morbidity of motor problems in specific disorders varied from almost one-fourth to more than one-third. The male/female ratio was significantly higher in children with motor problems and two or more other disorders, compared with children with motor problems and less than two other disorders. Conclusions; This study indicates that co-morbidity of motor problems with other clinical disorders is not exceptional and developmental deviance is seldom specific to one domain. However, current co-morbidity studies tend to overestimate the number of children with motor problems. In addition, there may be different patterns of symptoms between the genders. These findings stress the importance of assessing motor skills in children with various developmental disorders. Het gevaar van zulke verwarrende boodschappen is dat mensen die met kinderen in contact komen niet durven te vertrouwen op hun gevoel. Men moet geen deskundige zijn om signalen van kindermishandeling te zien. Het is trouwens een burgerlijke plicht om kinderen die zoiets meemaken, een hand toe te steken. Voor de ouder, vriend, leerkracht, arts, hulpverlener, student die hier werk van wil maken wil dit boek alvast duidelijkheid bijdragen. De jarenlange ervaring van het Vertrouwenartscentrum van de Universitaire Ziekenhuizen KU-Leuven wordt in dit boek samengebracht met steeds weer de vraag voor ogen "Hoe kijk je ernaar en wat kun je doen ?" De theoretische invalshoek geeft een korte stand van zaken weer, terwijl het accent ligt op de praktijkverhalen. Ieder hoofdstuk sluit af met een stappenplan dat voor de verschillende vormen van kindermishandeling schetst hoe je verder kan, vertrekkend van een vermoeden, van een signaal of van een verhaal van een kind.wonen.

Kindermishandeling komt veel meer voor dan men denkt. Binnen de opvoedingssituatie zijn vertrouwen en veiligheid nodig om met emoties, kennis en omgang met anderen te kunnen omgaan. Daarin zijn kinderen afhankelijk van volwassenen. Wat voor het vijfde levensjaar gebeurt, grijpt sterk in, temeer daar dan de belangrijkste fundamenten voor het persoon-zijn worden gelegd. Als geweld bij het opgroeiende kind plaatsvindt, heeft dat ook invloed op latere ontwikkelingen. Dit wordt in zeventien hoofdstukken en met veel voorbeelden toegelicht en besproken. Fysiek, psychisch en seksueel misbruik krijgen aandacht. Het boek kwam tot stand in samenwerking tussen een aantal psychotherapeuten, werkzaam bij het Vertrouwenscentrum Kindermishandeling (in 1987 door de Belgische regering officieel erkend) en gekoppeld aan de Universiteit van Leuven. Er is een trefwoordenlijst, een historische schets van het centrum en een aantal werkschema's opgenomen. Dit uitstekende boek is vooral geschikt voor ieder die met kinderen omgaat: hulpverleners, onderwijsmensen, justitiemedewerkers. En ouders die bewust bezig willen zijn om hun kinderen met liefde en vertrouwen naar de toekomst te begeleiden.

Des protocoles bien standardisés existent maintenant et ont prouvé leur efficacité pour réduire la mortalité associée aux cas graves d'amaigrissement. En revanche, les recommandations pour la prise en charge au niveau des services de santé de base n'ont guère changé, à l'exception de la prise en charge des infections infantiles. Ainsi, bien des occasions de repérer les cas et de les traiter à un stade précoce sont-elles perdues. Dans cet article, un cadre conceptuel est proposé pour mieux appréhender le processus et les différentes expressions de la malnutrition. Les modalités de prise en charge de la malnutrition, dans le cadre des programmes de santé à destination des enfants d'âge préscolaire, sont analysées sur cette base. On a en particulier cherché à identifier les lacunes et à élaborer des alternatives intégrant les services de santé de base dans une prise en charge plus globale. Ces services devraient ainsi être à même de jouer un rôle beaucoup plus important. In the case of growth faltering, this would trigger timely corrective measures. Although the relevance of growth monitoring and promotion (GMP) has often been questioned in the literature, opinions of District Medical Officers responsible for local implementation of GMP are unknown. The aim of this qualitative research was to explore the perceptions and difficulties of an international panel of District Medical Officers regarding GMP. As an exploratory study, in-depth interviews of an international panel of District Medical Officers (n=19) were conducted. Data were coded using the QSR Nudist 5.0 software. A discrepancy between intended purposes and practice of GMP was detected at two levels. First, lack of participation of care-givers was reported. Second, the District Medical Officers expressed a restrictive interpretation of the concept of growth monitoring. The communication with parents was never reported as a means or a result of GMP, neither as an evaluation criterion of programme efficiency. The growth chart was mainly considered a tool intended to be used by health services for the purpose of diagnosis. This two-fold discrepancy between the intention of international policy-planners and practice of local programme implementers could be a crucial factor affecting the performance of GMP. More emphasis should be put on social communication and involvement of caregivers.
