It has been hoped that regularly weighing children would result in the early detection of growth falterers, and that the growth chart would serve as an educational tool to make that state apparent to both health workers and caretakers in order to trigger improved caring practices. Our objective was to review whether GMP answers the theoretical grounds of a screening and intervention program. METHOD: A systematic literature review was performed. The WHO framework developed by Wilson and Jungner for planning and evaluating screening programs guided the analysis. RESULTS: Sixty-nine studies were retrieved. Overall, evidence is weak on the performance of GMP as a screening program for malnutrition through early detection of growth falterers. The main results are: (1) malnutrition remains a public health problem, but its importance is context specific; (2) the value of a low weight velocity to predict malnutrition is unknown and likely to vary in different contexts; (3) the performance of GMP for improving nutrition status of children and in reducing mortality and morbidity is unknown; (4) the performance of the screening is affected by the unreliability of weight measurements; (5) the promotional and educational effectiveness of GMP is low, in particular the growth chart is poorly understood by mothers; (6) the acceptability seems low in regards of low attendance rates; (7) evidence is lacking regarding cost-effectiveness. CONCLUSIONS: We conclude that there is too little scientific evidence to indiscriminately support international promotion of GMP. However GMP could constitute a valid strategy of public nutrition in specific situations. We indicate paths for further research and how prevention programs could be developed. Belgium ; Health Promotion ; Infant, Newborn ; Journal Article ; Peer Review ; Program EvaluationAbstract: This study evaluates the process of implementation of a longitudinal intervention program to promote oral health in preschool children in Flanders, Belgium. As the program was implemented in an existing preventive health care organization, the study also evaluates this setting as the context for implementation. Qualitative and quantitative methods were used to evaluate implementation fidelity, based on Carroll's theoretical framework of implementation fidelity (Carroll et al., Implementation Science 2:40, 2007). Questionnaire data from participants and health workers were analyzed, and document analyses were performed to compare registrations of the actions with the planning manual. Results were mixed. Whereas more than 88 % of all parents attended all home visits, only 57 % received at least 9 of the 11 planned consultations. Fifty-two percent of the families received all supporting materials, and on average, 73 % of all attending families received all information at a contact as described in the manual. Moderating factors such as the adequate use of facilitators and high participant responsiveness had a positive impact on implementation fidelity, whereas the quality of delivery differed to a great extent between the nurses who were involved during the entire intervention period and those who gave only a few sessions. Implementing an intervention in an existing well-baby program has many advantages, although lack of time presents a challenge to implementation fidelity. The results of this process evaluation allow a better understanding of the contribution of implementation fidelity to the effectiveness of health promotion programs. Belgium ; Breast Neoplasms ; Journal Article ; Mammography ; Mass Screening ; Program EvaluationAbstract: Introduction: In June 2001, a breast cancer screening programme was started in Flanders for all women aged 50-69 years. Until that date, screening was opportunistic and devoid of quality control. In this paper, the evolution during the first two-year round is compared with the situation before the programme.
Methods: The study population comprised all women aged 50-69 years living in Flanders in the period 2000-2003. The data source was the reimbursement claims database of all social insured of the country, for mammograms, breast ultrasounds and magnetic resonance imaging.
Results: In the 2002-2003 period, the participation rate by screening mammography was 32.7%. When adding the participation to the still existing opportunistic screening (21.7%), the percentage of women who underwent a mammography amounts to 54.3% of the study population, as compared to 40.5% in the preceding two years. The programme has also reduced inequities between age groups and socio-economic categories:
almost all women participate in an equal manner. In 2003, as
many as 77.3% of the diagnostic mammograms and 6.3% of the screening mammograms were followed within 90 days by additional imaging.
Conclusion: This organised screening programme already reached
a substantial proportion of the target group during its launch period. It also managed to attract women of all ages and of all socio-economic backgrounds. Yet, there remains a long way to go before reaching at least 70% of the target population.
