De plus en plus d’hôpitaux sont dans le rouge et le système n’est plus adapté aux besoins d’une population vieillissante. Le Centre Fédéral d’Expertise des Soins de Santé (KCE) a analysé les forces et les faiblesses du système actuel et a dessiné un nouveau cadre pour ses orientations futures. Les principaux points de friction du système actuel sont le sous-financement croissant, surtout en matière de soins infirmiers, l’offre excédentaire de lits et de services hautement spécialisés, les rétrocessions d’honoraires des médecins spécialistes hospitaliers, leurs inégalités de salaire et le manque de valorisation de la qualité.
Pour remédier à ces points problématiques tout en gardant le système abordable sur le plan des coûts, le KCE a développé un nouveau cadre global pour le financement futur des hôpitaux. Il comprend trois piliers principaux:
1 - Etendre et affiner le financement forfaitaire par admission existant, en le basant davantage sur les coûts réels, et créer un fonds d’investissement dans chaque hôpital, cogéré par les médecins et la direction ;
2 - Réformer la rémunération des spécialistes en rétablissant l’équilibre entre les revenus et les prestations réelles, et abolir les rétrocessions d’honoraires ;
3 - Adapter le type et le nombre de lits aux besoins de chaque “zone de soins” et réserver les soins complexes à des centres de référence spécialisés.
Il s’agit d’une proposition ambitieuse qui implique des changements profonds dans plusieurs secteurs des soins de santé. Meer en meer ziekenhuizen gaan in het rood, en het systeem is niet langer aangepast aan de noden van een vergrijzende bevolking. Het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) onderzocht de sterktes en de zwaktes van het huidige systeem en tekende een kader uit voor de toekomst. De groeiende onderfinanciering, vooral dan van de verpleegkundige zorg, het overaanbod van bedden en hooggespecialiseerde diensten, de afhoudingen op de erelonen van de ziekenhuisartsen, de ongelijkheid tussen hun erelonen en het niet belonen van een goede zorgkwaliteit en -coördinatie zijn de belangrijkste pijnpunten in het huidige systeem.
Om deze pijnpunten weg te werken en het systeem toch betaalbaar te houden, ontwikkelde het KCE een omvattend nieuw kader voor een toekomstige ziekenhuisfinanciering, gebaseerd op drie grote pijlers:
1 - Uitbreiding en verfijning van de bestaande forfaitaire financiering per opname, veel meer gebaseerd op reële kosten, en oprichting van een investeringsfonds in elk ziekenhuis, medebeheerd door de artsen en de directie.
2 - Hervorming van de vergoeding van de specialisten, met een herstellen van het evenwicht tussen het inkomen en de reëel gepresteerde inspanningen, en het afschaffen van de afhoudingen op de erelonen.
3 - Het aanpassen van het aantal en het type bedden aan de behoefte van elke ‘zorgregio’, en het voorbehouden van complexe zorg voor gespecialiseerde referentiecentra.
Het gaat alleszins om een ambitieus voorstel dat brede, ingrijpende veranderingen op veel verschillende vlakken vraagt.
