Mais il doute : en est-il de même pour un enfant d\'un an et demi ? Pour en être sûr, il va confronter ses connaissances avec ce qui est publié dans la littérature et disponible sur internet. Comment débuter cette recherche ? Dans ce premier article, le médecin découvre comment traduire sa question pratique en une question appelant une réponse, et, ensuite, en une question PICO. Ce dernier aspect permet alors au médecin généraliste de trouver des sources utilisables et pertinentes afin d’apporter une réponse rationnelle et fondée à sa question clinique. Frequent heavy drinkers place themselves and others at risk for a variety of adverse consequences and frequently remain undetected. Brief individual interventions result in a significant reduction on the number of drinks. Therefore, detection of students at risk is useful and desirable. The CUGE has been elsewhere described as a promising screening device for problem drinking in students. In order to determine the diagnostic value of this new questionnaire, we set up a validation study in a new and independent population of freshmen. METHODS: A cross-sectional diagnostic study. Participants were college freshmen of the Katholieke Universiteit Leuven. All students received a questionnaire, containing the CUGE, being the test of interest, and the CIDI as the reference test. RESULTS: The CUGE combines a very high sensitivity of 91% with a reasonable specificity of 76.3% in this validation group. CONCLUSIONS: The CUGE is an excellent screening device in this population of students. In addition, it is a short questionnaire with only yes or no questions. This makes the CUGE easily applicable as a part of broad routine questionnaires. Klinische tekenen uit anamnese en observatie hebben mogelijk belang om de kans op een ernstige infectie bij een kind in te schatten.
De gegevens van 384 kinderen, opgenomen op een spoedgevallendienst, werden geanalyseerd. Ernstige infecties werden gedefinieerd als sepsis, meningitis, pneumonie, pyelonefritis, bacteriële gastro-enteritis, osteomyelitis, bronchiolitis of cellulitis. Klinische en demografische variabelen werden prospectief geregistreerd en vervolgens multivariaat geanalyseerd.
25% van de kinderen in dit onderzoek had een ernstige infectie. De kans op een ernstige infectie was het hoogst bij kinderen jonger dan 1 jaar. De oddsratio (OR) voor kinderen van 1-3 jaar bedroeg 0,34 (95%-BI: 0,17-0,71), voor 4-6 jaar 0,18 (95%-BI: 0,07-0,45) en voor 7-16 jaar 0,36 (95%-BI: 0,15-0,87).
Kinderen verwezen naar de spoedgevallendienst door een andere arts vertoonden, in vergelijking met niet-verwezen kinderen, een OR van 2,2 (95%-BI: 1,1-4,1) voor een ernstige infectie.
De eerste indruk van de arts, vóór het uitvoeren van anamnese of klinisch onderzoek, dat het kind er hetzij tamelijk, hetzij zeer ernstig ziek uitzag, had een OR van respectievelijk 3,8 (95%-BI: 2,0-7,4) en 11,7 (95%-BI: 4,1-33,1).
