Maar een ziekenhuis is ook een bedrijf met een directie, managers en personeel dat goed bestuurd moet worden. Aangezien artsen vaak als zelfstandige werken in een ziekenhuis, wordt er weinig aandacht besteed aan een efficiënt hr-beleid. Toch kan een geïntegreerd ziekenhuis het zich niet veroorloven om niet beleidsmatig om te gaan met de inzet van en de relatie met haar artsen. Hoewel een eerste aanzet vaak zichtbaar is, hebben de meeste ziekenhuizen nog een extra duwtje in de rug nodig om echt werk te maken van een goed hr-beleid. Arts in het ziekenhuis toont directies en bestuurders de weg naar een succesvol artsenbeleid. Het boek geeft enerzijds een stand van zaken en denkt anderzijds na over hoe artsen in de toekomst op de meest adequate wijze ingezet en begeleid kunnen worden in een ziekenhuis. Arts in het ziekenhuis is het eerste Nederlandstalige boek dat dit thema uitvoerig bespreekt. Zowel het perspectief van de directie als het perspectief van de arts komt aan bod. WX 157.4 Multi-institutional systemsAbstract: In het kader van de hervorming van het ziekenhuislandschap bepaalde minister van Sociale Zaken en Volksgezondheid De Block onlangs dat ziekenhuizen meer structureel moeten samenwerken, om hun taken beter te verdelen en de zorg te coördineren. De huidige wettelijke samenwerkingsvormen en bestuursstructuren volstaan echter niet meer om deze doelstellingen te ondersteunen. Het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) stelt voor om drie nieuwe samenwerkingsvormen in de wetgeving op te nemen: het geïntegreerd zorgsysteem, het gecoördineerd netwerk en het autonoom samenwerkingsinitiatief. In elk van deze drie modellen worden de samenwerkende ziekenhuizen aangestuurd door een overkoepelend beslissingsorgaan, dat, afhankelijk van het gekozen model, een strategische of een meer beperkte beslissingsbevoegdheid heeft. Om een te bruuske breuk met de huidige situatie te vermijden, en om voldoende draagkracht voor de hervorming te genereren, stelt het KCE voor om de ziekenhuissector voldoende flexibiliteit te geven om zelf de meest geschikte samenwerkingsvorm en bestuursstructuren te kiezen. Objectives: This study aims to identify the facilitators and barriers to governance models of hospital collaborations. The country-specific characteristics of the Belgian healthcare system and legislation are taken into account.
Methods: A case study was carried out in six Belgian hospital collaborations. Different types of governance models were selected: two health systems, two participant-governed networks, and two lead-organization-governed networks. Within these collaborations, 43 people were
interviewed.
Results: All structures have both advantages and disadvantages. It is important that the governance model fits the network. However, structural, procedural, and especially contextual factors also affect the collaborations, such as alignment of hospitals’ and professionals’ goals,
competition, distance, level of integrated care, time needed for decision-making, and legal and financial incentives.
Conclusion: The fit between the governance model and the collaboration can facilitate the functioning of a collaboration. The main barriers we identified are contextual factors.
