W 265 Pharmaceutical insuranceAbstract: Het systeem van de artikel 81-conventie is een procedure voor de terugbetaling van geneesmiddelen die parallel loopt met de klassieke procedure. Ze treedt in werking wanneer er nog onzekerheden bestaan omtrent de werkzaamheid van een geneesmiddel, maar men het toch al beschikbaar wil maken voor de patiënt. Dit systeem bestaat in ons land sinds 2010, en aan het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) werd gevraagd om het te evalueren. Het blijkt dat de procedure steeds meer wordt gebruikt, maar dat het voordeel voor de maatschappij op lange termijn in vraag kan worden gesteld. Meestal worden bij het afsluiten van dergelijke vertrouwelijke overeenkomsten vertrouwelijke kortingen door de firma’s gegeven. Dit is uiteraard een voordeel voor de ziekteverzekering, maar een overmatig gebruik van het systeem leidt tot een verlies van transparantie en moedigt de farmaceutische firma’s niet altijd aan om bijkomend onderzoek uit te voeren naar de werkzaamheid van hun producten. W 265 Pharmaceutical insuranceAbstract: Le système de « convention article 81 » est une procédure d’accès au remboursement de médicaments qui est parallèle à la procédure classique. On la met en œuvre quand certaines incertitudes autour (notamment) de l’efficacité du médicament persistent encore, mais que l’on souhaite quand même mettre ce médicament à la disposition des patients. Ce système existe depuis 2010 dans notre pays et le Centre fédéral d’Expertise des Soins de santé (KCE) a été chargé d’évaluer son fonctionnement. Il en ressort qu’il est de plus en plus souvent fait appel à cette procédure, mais que ses bénéfices à long terme pour la société peuvent être mis en doute. En effet, de tels accords s’accompagnent généralement de ristournes confidentielles sur le prix du produit en question, ce qui est un avantage évident pour l’assurance maladie. Mais si l’on y recourt de façon excessive, cela mène à une perte de transparence de tout le système. Cela place aussi la barre moins haut pour les firmes pharmaceutiques en ce qui concerne la mise en route de recherches supplémentaires pour prouver l’efficacité de leurs produits.
