WX 186 Disaster planning and responseAbstract: Les incidents CBRNe sont des situations d’urgence caractérisées par la libération de substances chimiques (C) ou biologiques (B) ou par une contamination radiologique/nucléaire (RN), parfois par le biais d’une explosion (e). À titre d’exemples, on peut citer un accident dans une entreprise de produits chimiques ou lors du transport de substances dangereuses, une catastrophe nucléaire ou encore un attentat terroriste. Sollicité pour réfléchir aux moyens d’organiser encore mieux l’aide médicale dans ce type de situation, le Centre Fédéral d’Expertise des Soins de Santé (KCE) a consulté de très nombreux acteurs de terrain. Il plaide notamment en faveur du transfert de toutes les victimes grièvement blessées vers des hôpitaux spécialisés disposant d’une expertise CBRNe suffisante (les « hôpitaux CBRNe de Type I »). Les soins aux victimes moins grièvement blessées, eux, pourraient être administrés ou poursuivis dans les autres hôpitaux (dits « de type II »). Sur les lieux de la catastrophe, la mesure la plus importante est de procéder à une décontamination la plus rapide possible, doublée d’une aide médicale. Pour ce faire, il faudrait idéalement renforcer l’expertise médicale spécialisée (p.ex. équipes médicales d’urgence (SMUR CBRNe), conseiller médical en substances dangereuses, etc.) – et son déploiement rapide – sur le terrain. WO 704 BurnsAbstract: Les centres de traitement des grands brûlés parviennent de mieux en mieux à négocier la première phase, particulièrement critique, de la prise en charge des patients très gravement brûlés… mais une fois la personne sortie de l’établissement, les soins se déroulent d’une manière beaucoup moins structurée. A la demande de la Fondation des brûlés, le Centre Fédéral d’Expertise des Soins de Santé (KCE) a étudié comment mieux organiser ce volet de leur prise en charge. Les grands brûlés ne sont pas systématiquement traités dans un centre spécialisé, et ils ne bénéficient donc pas toujours de soins post-aigus parfaitement adaptés. La formation des urgentistes à l’identification des brûlures graves et des critères de transfert standardisés pourraient contribuer à remédier à ce problème. En outre, il est nécessaire d’harmoniser les procédures de sortie et d’organiser des soins post-aigus multidisciplinaires individualisés pour chaque patient. Afin de pouvoir référer adéquatement les patients vers les prestataires appropriés, une liste librement accessible des infirmiers, kinésithérapeutes et autres paramédicaux disposant d’une expertise dans le soin des brûlures graves devrait être établie. Il s’agit de personnes atteintes schizophrénie, de troubles bipolaires (aussi appelés troubles maniaco-dépressifs) et de dépression ou de troubles de la personnalité graves. En Belgique, l’approche thérapeutique consiste encore souvent en une prise en charge par une institution psychiatrique, alors que d’autres possibilités de soutien se sont développées.Le Centre fédéral d’expertise des soins de santé (KCE) recommande une réintégration plus importante de ces personnes dans la société, grâce à des soins de proximité dans leur lieu de vie. Une équipe multidisciplinaire composée entre autres d'un psychiatre, d’une infirmière psychiatrique, d’un assistant social, etc, devrait permettre une coordination des soins dans un certain nombre de cas.
