2011-01-01 ; Decision Making ; Prostatic Neoplasms ; R210 ; Watchful WaitingAbstract: Bij een lokale prostaatkanker kan er overwogen worden om eerst aan actieve opvolging te doen (‘Active Surveillance’ of AS).De tumor wordt dan regelmatig opgevolgd, en zodra hij evolueert wordt er een klassieke behandeling opgestart. De beslissing om een kanker niet onmiddellijk weg te halen is niet vanzelfsprekend. Het Federaal Kenniscentrum voor de Gezondheidszorg (KCE) ging na op welke manier artsen en patiënten kijken naar AS, welke factoren de patiënt beïnvloeden bij het aanvaarden van deze vorm van behandeling en wat artsen bereid maakt om ze aan te bieden. Uiteraard moeten de fysieke toestand van de patiënt, zijn leeftijd en de aard van de tumor geschikt zijn voor AS. Daarnaast spelen de voorkeur van de patiënt en diens omgeving, maar vooral de houding van de arts een grote rol. Een patiënt zal alleen dan voor AS kunnen kiezen als minstens één van de artsen die hij bezoekt overtuigd is van AS en hem geruststelt. Het KCE pleit voor een beslissing die door arts en patiënt wordt gedeeld. Dat vereist een volledig geïnformeerde patiënt en een arts met de juiste vaardigheden voor dit soort gedeeld beslissingsproces.

W 20 Graduate and continuing medical educationAbstract: A la demande de l’Agence Fédérale des Médicaments (AFMPS), le Centre fédéral d’expertise des soins de santé (KCE) a étudié l’impact des visiteurs médicaux indépendants et ce en collaboration avec les chercheurs de Spiral (ULG) et Medsat (Leuven). L’objectif de ces visites est d’offrir aux médecins généralistes une information scientifique indépendante. L’AFMPS a investi cette dernière année plus d’un million d’euros dans le projet. Le KCE n’a pu formuler de conclusion définitive au sujet de l’impact de l’initiative en Belgique, suite au manque de données disponibles. Le KCE recommande, si le projet est poursuivi, de mieux l’intégrer au sein des autres initiatives belges relatives à la qualité des soins. Par ailleurs il serait judicieux de mieux cibler le choix des sujets et le groupe de médecins qui recevront une visite. La méthode basée sur des contacts individuels devrait être complétée par d’autres moyens de communication. Enfin, un enregistrement correct de données couplé à leur évaluation est incontournable.
