En préparation de cette feuille de route, la ministre a demandé au Centre Fédéral d’Expertise des Soins de Santé (KCE) d’examiner le comment et le pourquoi de l’introduction d’un financement prospectif et forfaitaire dans 5 autres pays, et quelles leçons pouvaient en être tirées. L’expérience internationale nous apprend qu’il est très important de fixer à l’avance des objectifs clairs, et également de prendre des mesures accompagnatrices afin de corriger d’éventuels effets négatifs d’un financement forfaitaire. Un financement de ce type offre plus de transparence et une répartition plus équitable des ressources entre les hôpitaux. L’impact sur la qualité des soins et sur l’efficience n’est pas clair. Des mesures complémentaires sont nécessaires pour atteindre ces objectifs. Il s’agit d’un mode de rémunération dans lequel les hôpitaux recevraient un montant forfaitaire en fonction du nombre et de la nature des pathologies qu’ils prennent en charge. Plusieurs pays voisins appliquent déjà un tel système dans le but de responsabiliser financièrement les hôpitaux et d’accroître leur efficience. Le “All-in” est praticable en Belgique et préférable aux réformes fragmentaires et compliquées introduites jusqu’à présent. Cependant, le KCE recommande de rechercher un consensus préalable entre les parties prenantes et de n’introduire ensuite le système que de façon progressive.
