Artsen en ethici gaan in het boek op zoek naar het ‘Higgs-deeltje’ van palliatieve zorg: onzichtbaar maar essentieel voor de samenhang en het goede begrip.
Het goede levenseinde in casussen biedt vingerwijzingen aan iedereen die belang heeft bij geslaagde zorg. Omdat dit boek zich richt op de praktijk van de zorg, dus op het beslissen en handelen van zorgverleners en zorgvragers, begrijpen we de zorgpraktijk zowel van bovenaf, vanuit de theorie, als van onderaf, vanuit de praktijk. Uit de vele casussen blijkt dat er bij geslaagde zorg aandacht was voor wat ziek-zijn en afhankelijkheid voor een specifieke patiënt betekenen: de ene patiënt ervaart een gesprek over Advance Care Planning als een mededeling over een fatale diagnose, de ander als een blijk dat de arts zijn autonomie respecteert; de ene patiënt ervaart de optie van euthanasie als een troost, de ander meent daaruit op te maken dat hem een vreselijke natuurlijke dood te wachten staat; de ene familie ziet het staken van de beademing als medische bescheidenheid, de ander als een poging om iemand te doden. One of the purposes of this study is to describe which type of resources used in-hospital palliative patients and how their discharges were managed. Methodology: It was a prospective epidemiological study conducted in 14 randomly selected hospitals (4646 beds). All patients meeting the 'palliative' definition were included. The caregivers in charge of each patient were interviewed twice with a structured questionnaire. A part of these questionnaires were devoted to in-hospital resources used and to discharge's management. Results: 249 patients were included. Patient's characteristics and therapeutic strategies are described in abstracts I and II. Seventy percent of patients came from home. More than half were admitted through the emergency department. Sixty percent were hospitalized in acute beds and 31 % in geriatrics beds. Assistance of in-hospital palliative care teams had been requested for 31 % of patients and planned or considered for 13 %. These frequencies increased for younger patients, cancer patients, hospitalisation in acute-term care beds, when the survival prognosis was short and when the treatment's objective was to improve comfort. At the end of study, 17 % of patients had died in hospital, 32 % were always hospitalized and 51 % were discharged. Death concerned especially younger cancer patients and old women in geriatric beds, continuing hospitalization concerned younger patients whatever the pathology and older patients coming from home and discharges concerned cancer patients whatever their age and old non-cancer patients. Our study shows that different types of caregivers are involved in the treatment of palliative patients. This aspect should also to be taken into account when analysing or reforming the Belgian health system, specifically the palliative care services.
